Health professionals at ETHC maintain continuity of care with their patients. The providers engage with the patients through subsidized (charges may vary) home visits when there is one of the following:
• Recent discharge from hospital in high risk case
• Recent discharge from hospital that require daily assessment
• Severely incapacitated patients
• Patient requiring acute GP assessment
• Acute shortness of breath restricting mobility
• Care of terminally ill patient
